Are you interested in becoming a member of our rewards program? We are excited to offer our patients a unique whitening program.

When you come to our office for your preventive examination, x-rays and cleaning, we will provide you with custom whitening trays and materials for a one-time enrollment fee of $109.  Then, at each recommended preventive visit, we will give you a complimentary touch up kit of whitening gel.  This ensures that you will be able to keep your teeth bright and beautiful!

To be eligible for the program, you must meet the following criteria:

-  Be a New or Current patient of record
-  Have had a complete dental examination by Dr. Weisenfels with recommended x-rays.
-  Do not have active, untreated gum (periodontal) disease, decay (caries),     
   root sensitivity, oral cancer, etc.
-  Agree to read and sign whitening consent form.
-  Stay current in keeping with your recommended cleaning appointments (prophylaxis,   
    periodontal maintenance).

All we ask in return is:

-  You keep your six month (or 4 month) preventive visits current.  Your long term dental health is as 
    important to us as it is to you.  Our patients have found that these six month (or 4 month) visits help 
    greatly reduce emergencies.  That is why we are happy to provide this extra bonus for
    our patients who are committed to their dental health.

-  Provide at least 24 hours notice if you need to cancel or change an appointment.  In order 
   to provide exceptional services to all of our patients,  we ask that you give us the courtesy of advance   notice for schedule changes.
